SOCIAL WORKER PRN

Job Description

We are looking for a dedicated and compassionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a variety of common issues related to institutional living, family dynamics, healthcare understanding, aging, and end-of-life counseling.

Responsibilities

• Conduct comprehensive psychosocial assessments.
• Provide counseling and support to patients and families.
• Assist with discharge planning and address financial concerns.
• Collaborate with interdisciplinary teams to ensure holistic patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience.
