SOCIAL WORKER PRN

Job Description

We are currently looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a member of our Allied Health team, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Provide counseling and support to patients and families facing challenging situations.
• Conduct psychosocial assessments to identify patient needs and develop care plans.
• Collaborate with healthcare professionals to ensure holistic patient care.
• Assist in discharge planning and coordination of post-hospital care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Demonstrated knowledge of social work theory and practice.
