Job Description
We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will be responsible for providing clinical social work services and psychosocial assessments to patients/residents and their families, addressing a wide range of issues. As a Social Worker, you will play a crucial role in assisting individuals facing challenges related to institutional living, financial concerns, aging, medical procedures, discharge planning, and end-of-life care.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address various issues such as adjustment to institutional living, financial concerns, and family relationships.
- Offer counseling related to medical diagnoses, aging, discharge planning, and end-of-life care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of social work experience.
- LBSW, LMSW, or LGPC certification.
