SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker to join our team on a PRN basis at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing a variety of common challenges related to healthcare, aging, and personal well-being.

Responsibilities

• Provide psychosocial assessments and clinical social work services to individuals and families.
• Address issues related to institutional living, financial concerns, family dynamics, medical procedures, aging, discharge planning, and bereavement.
• Collaborate with interdisciplinary teams to develop comprehensive care plans.
• Offer counseling and support to individuals and families in times of need.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of relevant experience in a clinical setting.
• LBSW, LMSW, or LGPC certification.
• Strong interpersonal and communication skills.
