Job Description

We are looking for a compassionate and experienced Social Worker to join our team at LifeBridge Health. As a PRN Social Worker, you will provide vital clinical social work services and psychosocial assessments to patients, offering support in various areas such as adjustment, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Help patients navigate through medical procedures and treatments.
• Assist in discharge planning and support patients with aging-related changes.
• Offer support for emotional and financial challenges.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
