SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, families, and caregivers. The role involves addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to end-of-life care.

Responsibilities

• Provide psychosocial assessments to patients/residents and their families.
• Offer counseling and support for common problems faced by patients in a healthcare setting.
• Contribute to discharge planning and coordination of care.
• Collaborate with interdisciplinary teams to provide comprehensive care.

Requirements

• Basic professional knowledge and a Bachelor's degree.
• 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC licensure.
• Strong communication and interpersonal skills.
