Job Description
We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a wide range of challenges and concerns.
Responsibilities
- Provide support and counseling to patients and families facing various issues related to institutional living, financial matters, family dynamics, medical procedures, aging, powerlessness, and end-of-life care.
- Conduct psychosocial assessments to understand patients' needs and develop appropriate care plans.
- Collaborate with interdisciplinary teams to ensure comprehensive and holistic patient care.
- Participate in discharge planning and facilitate referrals to community resources.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of experience in social work.
- Licensed as LBSW, LMSW, or LGPC.
- Strong communication, empathy, and organizational skills.
