Job Description

We are looking for a knowledgeable and compassionate SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. This position involves providing clinical social work services and psychosocial assessments to patients/residents and families facing various challenges.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients/residents and families.
• Address common concerns such as adjustment to institutional living, financial issues, family relationships, and end-of-life counseling.
• Assist with discharge planning and counseling related to death and dying.

Requirements

• Bachelor's degree with 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and specialized field practices.
