SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD at Levindale Hospital. The role involves providing clinical social work services, conducting psychosocial assessments for patients and families, and addressing a variety of common issues such as adjustment to institutional living, financial concerns, and discharge planning.

Responsibilities

• Offer clinical social work services and psychosocial assessments.
• Address challenges related to adjustment, financial concerns, and family relationships.
• Assist with discharge planning and provide counseling on end-of-life issues.
• Collaborate with multidisciplinary teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree in social work.
• 1-3 years of social work experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
