Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and more.
Responsibilities
- Provide clinical social work services to patients and families.
- Conduct psychosocial assessments and address various emotional and social concerns.
- Assist patients with discharge planning and end-of-life counseling.
- Collaborate with healthcare professionals to ensure comprehensive care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of experience in social work.
- Licensed as LBSW, LMSW, or LGPC.
- Strong knowledge of social work theory and practice.
