Job Description

We are looking for a compassionate Social Worker PRN to join LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common issues such as adjustment difficulties, financial concerns, family relationships, aging-related changes, discharge planning, and counseling on end-of-life matters.

Responsibilities

• Offer clinical social work services and psychosocial assessments.
• Address a range of patient and family concerns.
• Provide counseling on various life challenges.
• Collaborate with healthcare teams to support patient well-being.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
