SOCIAL WORKER PRN

Job Description

We are looking for a talented PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address a range of common problems including adjustment difficulties, financial issues, and counseling related to death and dying.
• Collaborate with interdisciplinary teams for comprehensive patient care.

Requirements

• Bachelor's degree and 1-3 years of experience in the field.
• Licensure as LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
