SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a social worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face during their stay. This includes assistance with adjustment to institutional living, financial concerns, family relationships, counseling related to medical conditions and end-of-life care, discharge planning, and more.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
