SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face. This includes assisting with adjustment to institutional living, financial concerns, family relationships, counseling related to medical issues, discharge planning, and support related to end-of-life care.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Assist patients and families with adjustment to institutional living, financial concerns, and family relationships.
• Offer counseling and support regarding medical diagnoses, discharge planning, and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience as a Social Worker.
• LBSW, LMSW, or LGPC certification.
