Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. This position involves providing clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and address common problems patients face.
• Assist with discharge planning and counseling related to death and dying.

Requirements

• Bachelor's degree and 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
