SOCIAL WORKER PRN

Job Description

We are hiring a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. This position entails providing clinical social work services and psychosocial assessments to patients/residents and families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments of patients/residents and families.
• Provide counseling and support related to medical procedures and treatment.
• Assist with discharge planning and aging-related issues.
• Work collaboratively with healthcare team members to ensure comprehensive care.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of experience in a similar role.
• Licensed as LBSW, LMSW, or LGPC.
• Strong interpersonal and communication skills.
