Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. This role involves providing clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, and discharge planning.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and their families.
• Address common problems including adjustment to institutional living, financial concerns, and family relationships.
• Assist with discharge planning and counseling related to death and dying.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
