Job Description
We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a member of our Allied Health team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to medical conditions and end-of-life care.
Responsibilities
- Conduct psychosocial assessments and provide clinical social work services to patients and families.
- Address common issues related to institutional living, financial concerns, aging, discharge planning, and end-of-life care.
- Collaborate with the healthcare team to ensure holistic patient care.
Requirements
- Bachelor's degree with 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
- Strong interpersonal and communication skills.
- Compassionate and empathetic approach to patient care.
