SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a variety of common challenges. From adjustment to institutional living to counseling related to death and dying, you will play a vital role in supporting individuals through difficult circumstances.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common problems such as adjustment to institutional living, financial concerns, and family relationships.
• Offer counseling related to medical diagnoses, aging, powerlessness, and end-of-life care.
• Participate in discharge planning activities to ensure smooth transitions for patients and residents.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong interpersonal skills and a compassionate approach to patient care.
