Job Description
We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As part of the social work team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. Your responsibilities will include addressing various issues such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and grief counseling. This position is on an as-needed basis from 8:30 am to 5:00 pm.
Responsibilities
- Provide clinical social work services to patients and families.
- Assess and address common psychosocial issues related to medical treatments, aging, and end-of-life care.
- Collaborate with healthcare teams to develop effective care plans and discharge strategies.
Requirements
- Minimum Bachelor's degree with 1-3 years of relevant experience.
- Licensed Social Worker (LBSW/LMSW/LGPC)
- Strong knowledge of social work theory and practice.
