HOSPICE MEDICAL SOCIAL WORKER

Job Description

We are currently seeking a compassionate and experienced Hospice Medical Social Worker to join our dedicated team at Liberty Hospice. As a Hospice Medical Social Worker, you will provide direct patient care services, conduct clinical assessments, develop individualized care plans, and offer emotional support to patients and families during challenging times.

Responsibilities

• Provide direct patient care services and document patient care data.
• Conduct clinical assessments and create individualized care plans.
• Apply psycho-social principles to understand and address social, emotional, financial, and environmental factors affecting patient treatment.
• Coordinate with agency personnel and other services to ensure optimal care.
• Assist with homecare visits and provide bereavement support as needed.
• Educate patients and families on community resources and support services.

Requirements

• Master's degree in Social Work from an accredited institution with at least one year of practical experience in home health or hospice care.
• Strong interpersonal skills and ability to communicate effectively in a multidisciplinary environment.
• Willingness to travel and work in various conditions as assigned.
• Completion of at least twelve hours of continuing education annually.
