INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. In this role, you will collaborate with the clinical team and medical provider to provide discharge planning coordination and intervention for patients with complex health needs.

Responsibilities

• Coordinate with the clinical team and medical providers for efficient patient care.
• Promote patient wellness and improve care outcomes.
• Ensure the efficient utilization of health services.
• Minimize denials of payment among patients with complex health needs.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• BSN required from CCNE accredited schools.
