INPATIENT CASE MANAGER (RN)

Job Description

We are looking for an experienced Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position based in Randallstown, MD at Northwest Hospital offers a sign-on bonus potential of $15,000. As an Inpatient Case Manager, you will work collaboratively with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs.

Responsibilities

• Provide discharge planning coordination to facilitate patient wellness and improved care outcomes.
• Efficiently utilize health services and minimize payment denials for patients.
• Collaborate with the clinical team to promote efficient patient care.

Requirements

• Bachelor's degree in Nursing (BSN) from CCNE accredited schools.
• Professional knowledge and working experience in a specialized field.
• Prior experience as a Utilization Review Nurse, Clinical Care Coordinator, Patient Care Integrator, or RN Case Manager at LifeBridge Health is a plus.
