Job Description

Curana Health, a provider-led primary and post-acute care organization dedicated to senior living communities and residents, is seeking a Nurse Practitioner or Physician Assistant to join their team at Springfield, MA. As an essential member of the healthcare team, you will provide direct patient care services within the Post-Acute Care setting, focusing on quality care delivery and patient autonomy.

Responsibilities

• Conduct clinical direct care visits, evaluations, and treatment of chronic illnesses.
• Collaborate with physicians for medical management and coordination of care.
• Perform Annual Wellness Visits and coordinate care with other healthcare disciplines.
• Maintain effective communication with families and attend conferences when possible.

Qualifications

• Valid nurse practitioner or physician assistant license with current board certification.
• Masters Degree in Nursing or Physician Assistant Studies.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills with the ability to work within a team.
