Job Summary:

The Post Permanency Coordinator will ensure compliance with Adoption State and County regulations for children, youth, and families referred through the Statewide Adoption and Permanency Network (SWAN) to APM. Responsibilities include case management for permanency services, assessment of resource homes, documentation compliance, progress monitoring, training assistance, and attendance at regional and statewide meetings.

Requirements:

  • Completed Bachelor's Degree in Social Work, Psychology, or related field.
  • Minimum one year working in adoption systems or direct social service delivery.
  • Knowledge of PA Chapter 3700 preferred.
  • Familiarity with Microsoft Office Suite.
