Purpose
The Social Worker at Northwest Kidney Centers is responsible for providing social work intake, patient advocacy services, and coordinating social services for patients, families, and other organizations. This role includes patient education and counseling, as well as serving as a resource to employees and participating in Quality Assessment Performance Improvement initiatives.
Responsibilities
- Conduct comprehensive social service needs assessments for each patient
- Develop individualized care plans
- Educate patients on their rights and grievance process
- Collaborate with interdisciplinary team members
- Participate in Quality Assessment Performance Improvement activities
Requirements
- Master of Social Work (MSW) degree
- Valid Washington Social Worker License
- Previous experience in medical social work preferred
- Valid Washington Driver's license, reliable transportation, and car insurance
