Job Description

We are looking for a dedicated Case Manager to join House of Hope Green Bay, Inc. As a Case Manager, you will play a vital role in providing essential services and support to clients in our emergency shelter and housing stability programs. Your responsibilities will include conducting case management activities, coordinating with local agencies, maintaining client records, and ensuring the success of our clients.

Responsibilities

  • Provide case management services to assigned clients according to program guidelines.
  • Collaborate with team members to plan and implement effective client services.
  • Screen clients for program eligibility and compliance with regulations.
  • Maintain accurate client records and case notes.
  • Ensure a safe and supportive environment for clients in the emergency shelter.
  • Participate in crisis intervention and program development.

Requirements

  • Valid Wisconsin driver's license and successful completion of a background check.
  • Degree or experience in social work, psychology, or related field.
  • Ability to work with diverse populations and effectively communicate with others.
  • Knowledge of local housing programs and community resources preferred.
  • Strong organizational skills and the ability to work well in a team setting.
  • Physical and mental ability to meet the demands of the position.
