Job Description
Heartland Home Health, part of the Gentiva family, is seeking a dedicated RN Case Manager to join our team. As a Home Health RN Case Manager, you will provide skilled nursing care to clients in their homes, ensuring quality services and support to patients and their families.
Responsibilities
- Administer skilled nursing care as per physicians' orders.
- Teach and supervise family members and nursing team.
- Ensure services are delivered in compliance with regulations and standards.
- Ability to travel extensively on a daily basis.
Requirements
- Active RN license in state of practice.
- Minimum of 6 months recent nursing experience.
- 2 years of RN experience for PASSPORT patients.
- Strong interpersonal and communication skills.
