Job Description
We are looking for a dedicated Hospice Social Worker to join our team at LifeBridge Health. This part-time position, specifically for weekends, offers the opportunity to make a meaningful impact on individuals and families in the home setting. As a Hospice Social Worker, you will assess psychosocial, environmental, and financial needs, collaborate with patients and families on care plans, provide counseling services, and coordinate necessary referrals.
Responsibilities
- Assess patients' and families' psychosocial, environmental, and financial needs.
- Collaborate with patients, families, and caregivers to develop and evaluate comprehensive care plans.
- Provide counseling services to support patients and families during the dying and grieving process.
- Coordinate with community and agency referrals for additional services.
Requirements
- Master's Degree in Social Work from an accredited school.
- 2-4 years of clinical experience, with one year in a community health setting preferred.
- LCSW or LCPC certification.
- Professional licensure in Maryland and Pennsylvania.
