INPATIENT CASE MANAGER (RN) - PRN

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. In this role, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions. The ideal candidate will focus on promoting patient wellness, improving care outcomes, and optimizing health service utilization for patients with complex needs.

Responsibilities

• Coordinate discharge planning in collaboration with clinical team and medical providers.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Work to minimize denials of payment among patients with complex health needs.

Requirements

• Bachelor's degree with a nursing background.
• 3-5 years of relevant experience.
• American Heart Association CPR Certification.
• Maryland Registered Nurse License.
• Case Management certification required within 3-5 years of hire.
