INPATIENT CASE MANAGER (RN)

Job Description

We are seeking a dedicated Inpatient Case Manager (RN) to join our dynamic team at LifeBridge Health. In this role, you will collaborate with the clinical team and medical providers to deliver exceptional discharge planning coordination and intervention services. The primary focus of the position is to promote patient wellness, enhance care outcomes, optimize health service utilization, and minimize payment denials within a patient population with complex health needs.

Requirements

• Bachelor's degree in a related field; BSN preferred, ADN required.
• 3-5 years of relevant experience.
• Demonstrated knowledge in specialized fields.
