INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Case Manager, you will work collaboratively with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, improving care outcomes, and ensuring the efficient utilization of health services.

Responsibilities

• Coordinate discharge planning in collaboration with the clinical team and medical provider.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment for patients with complex health needs.

Requirements

• Bachelor's degree in a relevant field; BSN preferred, ADN required.
• Maryland Registered Nurse License.
• Case Management certification required within 3-5 years of hire.
• Experience in care coordination and discharge planning preferred.
