Job Description
We are looking for a dedicated and experienced Inpatient Case Manager - Social Worker (LMSW) to join our team at LifeBridge Health. In this full-time position at SINAI HOSPITAL in Baltimore, MD, you will be responsible for providing patient and family advocacy, discharge planning coordination, and psychosocial intervention for high-risk inpatients. Your role will focus on promoting patient and family wellness, improving care outcomes, and ensuring access to appropriate hospital and community resources for patients with complex health needs.
Responsibilities
- Collaborate with the clinical team and medical provider to advocate for patients and families.
- Coordinate discharge planning and ensure seamless transitions for inpatients.
- Provide psychosocial interventions to support patient well-being.
- Promote access to relevant hospital and community resources.
Requirements
- Master's degree in Social Work.
- 2+ years of experience in hospital social work or community social services.
- LMSW required; LCSW/LCSW-C preferred.
- Valid MD Social Work License.
- Strong verbal and written communication skills.
