Job Description
We are looking for an Inpatient Case Manager - Social Worker (LMSW / LCSW-C) to join our team at LifeBridge Health. This is a full-time position based in Baltimore, MD at SINAI HOSPITAL. As an Inpatient Social Worker, you will collaborate with the clinical team and medical provider to provide patient and family advocacy, discharge planning coordination, and psychosocial intervention for high-risk inpatients. Your role will focus on promoting patient and family wellness, improving care outcomes, and ensuring access to appropriate hospital and community resources for patients with complex health needs.

Requirements
- Master's in Social Work Required.

- MD Social Work License per level of education.
