Summary
Job Summary: Provides clinical social work services and psychosocial assessments to patients/residents and families. Addresses issues such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and counseling on death and dying.
Requirements:
- Bachelor's in Social Work (BSW), Master's degree preferred
- LCSW-C certification preferred
- LGSW certification
