INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated and compassionate Inpatient Case Manager (RN) to join our team at LifeBridge Health. This is a full-time position located at Northwest Hospital in Randallstown, MD. As an Inpatient Case Manager, you will work collaboratively with the clinical team and medical provider to provide discharge planning coordination and intervention for patients with complex health needs.

Responsibilities

• Promote patient wellness and improved care outcomes.
• Utilize health services efficiently and minimize payment denials.
• Provide discharge planning coordination.
• Collaborate with clinical team and medical provider.

Requirements

• BSN required from CCNE accredited schools.
• Knowledge of theory and practice within a specialized field.
• Basic professional knowledge equivalent to a Bachelor's degree.
