Job Description

We are looking for a dedicated RN Care Coordinator to join our Ambulatory Care team at Denver Health. As a RN Care Coordinator, you will play a vital role in coordinating patient care through collaboration with multidisciplinary teams and patients/families to ensure quality outcomes within care coordination parameters. You will assess, plan, support, and evaluate patient care, facilitate transitions of care, and contribute to the development of care systems for specific patient populations. This position involves face-to-face, telephonic, and electronic interactions with patients across diverse settings.

Requirements
