Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a vital member of our Allied Health team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of issues from adjustment to medical concerns to end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide support to patients and families.
- Assist with discharge planning and counseling on aging and end-of-life matters.
- Work collaboratively with medical staff to address patient needs.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
