Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a full-time Social Worker, you will provide clinical social work services to patients/residents and families, addressing various challenges and concerns they may face. This position involves offering support and guidance on issues such as adjustment to institutional living, financial matters, family relationships, aging-related changes, discharge planning, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments.
- Provide counseling and support to individuals and families.
- Assist with discharge planning.
- Collaborate with medical teams to address patient needs.
Requirements
- Bachelor's in Social Work (BSW) required. Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) certification preferred.
- Licensed Graduate Social Worker (LGSW).
