As a crucial member of the team, you will provide clinical social work services to patients and families, addressing various social and emotional challenges. This role involves conducting psychosocial assessments, assisting with discharge planning, and providing counseling on end-of-life issues.
Responsibilities
- Provide clinical social work services to patients and families.
- Address a wide range of social problems faced by patients.
- Assist with discharge planning and end-of-life counseling.
Requirements
- Bachelor's in Social Work (BSW), Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW).
