INPATIENT CASE MANAGER (RN)

Job Description

We are looking for an Inpatient Case Manager (RN) to join our dedicated team at LifeBridge Health. As an Inpatient Care Manager, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, improving care outcomes, ensuring efficient health service utilization, and minimizing payment denials.

Responsibilities

• Provide discharge planning coordination for inpatient care.

Requirements

• Bachelor's degree in a related field.
• BSN required from CCNE accredited schools.
• Prior experience in utilization review, clinical care coordination, or case management is a plus.
• Strong communication and collaboration skills.
